Polish, Czech, and Sorbian: nickname derived from miły (Polish and Sorbian) milý (Czech) ‘gentle dear; pleasant’. As a Polish and Sorbian surname it may also be from a pet form of a personal name containing the Slavic element mil ‘gentle dear’.
